1. Promoter

The promoter is FORM Daily Co (“FORM”), operated in Australia.

2. Eligibility

Entry is open to Australian residents aged 18 years or over only.

Residents of South Australia (SA) and the Australian Capital Territory (ACT) are excluded from this promotion due to local trade promotion permit requirements.

Eligible states and territories include:

• Victoria (VIC)
• New South Wales (NSW)
• Queensland (QLD)
• Western Australia (WA)
• Tasmania (TAS)
• Northern Territory (NT)

Employees, immediate family members, or anyone professionally associated with the promotion are not eligible to enter.

3. Promotion Period

The promotion opens upon publication on the FORM website and closes at 11:59pm AEST on 13 September 2026.

4. Prize

One (1) winner will receive:

• AUD $5,000 cash paid via bank transfer to the winner’s nominated Australian bank account.

The prize is not transferable or exchangeable.

5. How To Enter

Eligible customers will automatically receive entries through qualifying purchases made via the official FORM website during the promotion period.

Entry allocation:

• 1 pack purchased = 1 entry
• Bundle purchases receive entries equal to the number of packs included
• There is no maximum number of entries per person

Additional review bonus entries:

Customers who leave a genuine product review containing either:

• a photo, or
• a video

will receive one (1) additional bonus entry per verified review submission.

Reviews may be positive or negative. FORM will not alter eligibility based on review sentiment.

6. Entry Tracking

Entries are tracked automatically using the customer’s order information, including name, email address, and order number recorded at checkout.

Review bonus entries are tracked using the email address associated with the verified purchase and review submission.

7. Refunds & Returned Orders

Any order that is refunded, charged back, cancelled, or returned before the draw date will have all associated entries voided and removed from the promotion.

8. Winner Selection

The winner will be randomly drawn on 20 September 2026.

The draw will take place electronically using a random selection method from all valid entries received during the promotion period.

9. Winner Notification

The winner will be notified by:

• email, and
• announcement on FORM social media platforms

on or after 20 September 2026.

If the winner cannot be contacted within 14 days, FORM reserves the right to redraw the prize.

10. Verification

FORM reserves the right to verify the validity of entries and entrants, including proof of identity, age, residency, and purchase.

FORM may disqualify any entrant suspected of:

• fraudulent activity,
• manipulating the promotion,
• providing false information, or
• breaching these Terms & Conditions.

11. Liability

To the maximum extent permitted by law, FORM is not responsible for:

• technical issues,
• delayed entries,
• lost entries,
• website interruptions,
• banking transfer errors outside of FORM’s control, or
• any indirect or consequential loss associated with the promotion.

12. Privacy

By entering this promotion, entrants agree that FORM may collect and use personal information for the purpose of administering the giveaway, contacting winners, and verifying entries in accordance with FORM’s Privacy Policy.
