FORM. 12 Week Glow Guarantee
1. Eligibility
The guarantee applies to all customers who have purchased any FORM. Daily Glow Gummies product directly from formdailyco.com.
2. Conditions
To be eligible for a refund under this guarantee, the customer must:
- Have taken two FORM. Daily Glow Gummies every day for a minimum of 12 consecutive weeks.
- Provide a dated before photo taken on or before day one of use, in natural bright light with the date clearly visible.
- Provide a dated after photo taken at or after week 12, in the same natural light conditions with the date clearly visible.
- Submit both photos to hello@formdailyco.com with their original order number.
3. Refund
If the conditions above are met and FORM. determines no visible difference is present, a full refund will be issued to the original payment method within 5–10 business days.
4. Exclusions
The guarantee does not apply to:
- Customers who have not completed the full 12 week course consistently.
- Photos that do not meet the requirements above.
- Orders purchased from third party retailers.
- Customers who have previously received a refund under this guarantee.
